Activity Parent Meeting

High School Commons
The 2010-11 activity meeting for ALL Students and Parents who will be participating in ANY extra-curricular activity in the upcoming school year. You must print off all forms needed below and bring them to this meeting. ***By attending this one meeting parents will be eligible for a reduction in their family max by $50. This meeting will only be offered on this date!
The handbook and individual forms are attached below. Please print off these forms and bring them to the meeting signed by a parent/guardian and student.
Physicals form is due once every 3 years
